Yellow Dot is a program that was first started in Connecticut in 2002 to help drivers share critical information with first responders at the scene of a crash when they may be unable to communicate for themselves.
We encourage everyone in Lee County to take advantage of this wonderful program. It takes only a few minutes to fill out the Yellow Dot form, and you can do it at your leisure. This simple, important program could save your life during that “Golden Hour” following an accident.
While we all hope and pray that none of us are ever involved in an accident but we also know that they happen. This program provides information that may be critical to your care to the first responders. There is no age limit for this program and we are asking that ALL citizens of Lee County sign up.
We are also available to come to your group meetings to talk about the program and we can sign people up while we are there.
Your information stays with you
One very important thing to note about this program is that no information on any individual is collected or stored anywhere. You fill out all the vital information, place it in your vehicle’s glove compartment, and you maintain it.

You can pick up your Yellow Dot folder at the Lee County Sheriff’s Office (1900 Frederick Rd., Opelika, AL 36801) or at our booth at community events.

I no longer drive. Can I still participate in Yellow Dot?
Yes. The decal and folder can be placed in the vehicle(s) you are normally a passenger in. You can also request an extra folder to keep in a purse, etc. to keep with you when riding in additional vehicles.
What if I drive a motorcycle?
Let us know that it’s for a motorcycle when you pick up your Yellow Dot folder so that we can make sure to give you the smaller sticker that we have just for motorcycle windshields. Then be sure to put the Yellow Dot folder in the same spot on your motorcycle you keep the rest of your paperwork.
I wish my mother were here, this program would be great for her.
We will give you a folder, decal, information sheet and brochure for your mother or other family member/friend. They can fill it out, attach a photo to the form, and put it in their car. They do not have to be present with you to get the yellow dot folder/form.
I am not a senior citizen, can I still participate?
Yes! Yellow Dot is being offered to anyone of any age. We encourage everyone to participate.
I don’t have any medical conditions and am on no medication, should I still participate?
Yes, Yellow Dot will still benefit you. First responders would have a list of your emergency contacts. Be sure to write the word “none” under medical conditions, allergies and medications. This will enable first responders to know that there are no pre-existing medical conditions that need to be considered when treating you.
Will you keep my information?
No. All of your information goes with you and is kept in your vehicle.
I don’t want to have my picture taken today.
No problem, we will give you everything that you need to participate in the program and you can attach a photo of your own.
I don’t have a way to print my picture at home.
Many pharmacies will print photos for you at a fairly low cost. Some even have an online upload and in-store pickup option so you can get your photo on the way home from work. We can also print a Polaroid photo for you at our office if needed.
Can I place Yellow Dot in my RV or boat?
Yes, in fact you can affix the sticker to your house if you desire.
If I trade my vehicle in or wreck my vehicle, how do I get another decal?
Simply stop by our office and ask for a new decal. You can use it with the same folder/information sheet that you have, you do not have to get a new sheet.
My list of medical conditions/medications will not fit in the space allowed.
If you have your information with you, we can photocopy it and paperclip it to your medical information sheet, if not just make a copy and put it into the folder, making sure to note on the provided medical form that there is additional information.
This would be great for my brother who lives in _______ County, what other counties are participating?
We recommend calling your county’s Sheriff’s Office or other law enforcement office to see if they are participating in the Yellow Dot program and requesting that they add the program if they do not already.
Current Alabama counties include: Chambers, Etowah, Jackson, Mobile. Alabama cities include but are not limited to: Demopolis, Notasula, Troy.
I don’t live in Lee County can I still sign up?
Definitely! However, the first responders in your county may or may not have heard about the program. It will still be available for their use if you are able to direct them to the glove box or if they look there for information.
What other states is this program in?
Connecticut, Florida, Georgia, Iowa, Massachusetts, New York, Tennessee, Virginia, West Virginia, and more. Please note that the first responders in all counties/cities may not be familiar with the program as in some places it is done on a state level, and in others it is done on a local level.
